Axa Health has increased the fees it pays for medical procedures by 15% as some doctors are threatening to “decouple” from the insurer if it does not meet their higher self-pay rates. Sarah Taylor, head of specialist and practitioner strategy at Axa Health, admitted some of the fees previously paid were “woeful” as she revealed
