Legal & General partners with long-term condition management service

Legal & General Group Protection has agreed a partnership with long-term condition management support service ConnectPlus, from Health and Care Innovations (HCI).

Available to all insured employees and their immediate family, the ConnectPlus app helps support the day-to-day self-management of six long-term conditions – stroke, cancer, multiple sclerosis, Type 2 diabetes, long Covid and rheumatoid arthritis.

The insurer said the evidence-based app will play a central role in its personalised multidisciplinary team (MDT) pathways, helping individuals to take control of their condition and improve their health – both at home and at work.

It was designed alongside NHS clinicians and patients – and is used in various NHS Foundation Trusts. The app includes:

Legal & General added the app also benefitted line managers and HR by helping them understand a condition and its impact on life and work ability, so they can better support employees, including making informed reasonable adjustments where required.
